It is called an Addictions Specialist.

From Medical Dictionary:

“(1) Addictionologist
(2) A general practitioner thrust in the role, when one of his/her patients suffers an addiction, or an allied health care professional—e.g., a nurse or psychologist—functioning in the context of an ‘outreach’ program”
